The Care Connect Notebook is a practical, youth‑friendly tool designed to help adolescents and young adults with cancer feel more prepared, confident, and supported during their healthcare journey.
Created in partnership with young people with a lived experience of cancer and Youth Cancer Services, this notebook helps to organise appointments, track treatments and medications, and write down questions, concerns, and notes before and after visits. It prompts important conversations about what matters to young people going through cancer treatment.
